Preferences only apply when you have a work sheet open. I was having problems saving files as the 'Save' option was greyed out. So I was checking preferences without a worksheet open. Update of Excel and High Sierra seem to have stabilised the network connection which I believe caused the issue. Just tried something:
My colleague has the same Mac and OS as me, I took the 'Mail' folder from their Library as well as the com.apple.mail.plist file from the Preferences and replaced mine.
After a restart I ended up with their e-mail (as you would expect) but this time the Add Account was not greyed out.
We will help you update your account settings. When your create your Spotify account through Facebook, the account settings get automatically filled in. This is why, they can't be changed by you afterwards. We would suggest for you to go here and open our support page. The folks there can help you change the settings. I had the same issue with a Konica Minolta Bizhub C652 running Sierra 10.12.4. The save settings box for account track is greyed out in Acrobat, but I was able to save the settings using the 'Print using system dialog' option in Chrome and save it as a preset that applies to all programs. Normally they are both greyed out if the padlock is locked or both able to be set or unset. These attributes are saved in your screen saver preferences and system preferences plist file located in /Library/Preferences/com.apple.screensaver.plist (the 'Requires password' and time settings) and /Library/Preferences/com.apple.systempreferences.plist (the disable automatic login setting).
However, clicking it still does nothing, and the + in the Preferences Accounts also still does nothing.
I was able to edit my colleagues account though, but this is not ideal, and I still can't add one of my own.
The next thing I did was delete the com.apple.mail.plist file and restart to see if Mail creating it's own file would now work. It didn't. It created one but Accounts were greyed out again. So i've replaced my colleagues one again.

I just tested a new installation of the 364e driver using the bizhub_554e_109.pkg installer and was able to open the Output Method menu and select the Account Track checkbox and then enter a Department Name, a password and press the Save Settings checkbox as shown below.
If you haven't done so already, run the reset of the printing system again and then trash the KonicaMinolta folder in /Library/Printers. This folder contains the plugins for performing these KM functions and the user may have restrictions on access - possible if the files were installed for a previous version of OS X on this Mac and then upgraded to Sierra. With the folder removed you can then run the v1.09 installer again and add the printer.
As for CUPS not showing the Account Track setting, the function relies on a KM plugin rather than it being a PPD option, hence your ability to set or save account information will not appear in the CUPS Set Printer Options settings.
